Notes

This example is designed to demonstrate the use of MailForm. Illustrated are the abilities to use text input fields, drop-down selection lists, check boxes, and multi-line input text areas.

When you submit this example form, this page is designed to e-mail you the results of the form (which would ordinarilly be sent to someone else); the intent being to allow you see firsthand what the results look like before you download and install MailForm. To do this, this web page uses an extremely short JavaScript routine which should work in Netscape Navigator 2.0 or higher or Microsoft Internet Explorer 3.0 or higher. This routine is in no way related to the function of MailForm and is only present to prevent you from having to type your e-mail address twice (once for to indicate who the e-mail is from, and once to indicate who it is to). If your web browser does not support JavaScript, the form will be e-mailed to myself, kbyanc@posi.net, and I will forward it back to you as soon as possible.
